POSITION: Social Services Director
Geary Community Nursing Home
LOCATION: Economic Development, Concho , Oklahoma

Description Summary: Must be able to assess and identify the social needs of each resident. Must have knowledge of current federal and state regulations.
DUTIES AND RESPONSIBILITIES:

Plan, develop organize, implement, evaluate and direct the Social Service programs. Coordinate Social Service activities with other departments.

Interview each resident/family prior to or within seventy two (72) hours of admission to obtain background history, make initial assessment, and develop a social service plan of care.

Develop and maintain a good working rapport with staff, medical profession and consultants, outside community health, welfare and social agencies to assure that social service programs can be properly maintained to meet needs of residents. As a member of interdisciplinary team, identify and seek ways to support resident’s individual needs and preferences, customary routines, concerns and choices.

Make referrals and obtaining services from outside entities (e.g., talking books, absentee ballots, community wheelchair transportation, etc.) Make arrangements for obtaining needed adaptive equipment, clothing and personal items. Assist residents in utilizing individual and group activities to the best advantage. Counsel residents. Assist residents with financial and legal matters.

Maintain confidentiality of all pertinent resident care information to assure resident rights are protected.

Observe infection control and safety policies and procedures at all times.
QUALIFICATIONS:
- High school diploma or GED preferred.
- Must have either a certificate of Social Service/Activities training approved by the state of Oklahoma for nursing homes or be able to successfully complete training approved by the Oklahoma State Department of Health.
- Must be able to read, write and speak the English language.
- Two years experience in Long Term preferred.
- Must have some understanding of the social psychological needs of the aged.
- Must pass OSBI background check and pre-employment drug screening.
SALARY: Commensurate with Education and work experience.
