2008 Gaming Per Capita Schedule

v    Your complete notarized W-9 and Change of Information must be in the Per Capita Office in order for your check to be printed.

v    ALL CHECKS WILL BE MAILED to the W-9 address. Checks will not be handed out.

v    Outstanding December per capita checks will not be reissued during August 2008.

v    There will be no holds or stop payments taken before August 25 without a police report, proof of filing a court order in Cheyenne and Arapaho Tribal Court, or a Cheyenne and Arapaho Tribal Court order.

v    If you know that your check will return, mail your original notarized fonns to Per Capita.

v    If your check returns & you wish someone else to pick up your check, your written request must be notarized with your signature & submitted with your change of address forms.

Last day to submit both forms for first check run Thursday, July 31

All original forms accepted by mail & walk-in until August 1.

Per Capita closed for processing August 1 through 22

All original forms accepted by mail only until August 25.

Last day to claim 2007 Gaming Per Capita Thursday, August 14

First check run mailed Friday, August 15

First day to verify whether check is returned Monday, August 25

First day to submit regular Stop Payment requests.

Checks will be reissued & mailed in September.

All original forms now accepted by mail & walk-in.

Checks mailed no later than Friday, August 29
