POSITION:  Optometry Technician

LOCATION:  Diabetes Wellness Program, Concho, (travel to Clinton)

DUTIES AND RESPONSIBILITIES:  This position shall be under the supervision of the Project Director for the Cheyenne-Arapaho Tribes Diabetes Wellness Program.  The purpose of  this position is to assess, support and implement eye care and services plans for clients of the Wellness Program.  Services will be assessed according to the needs of the target communities adapting them to fit their cultural, social and economic conditions.

MAJOR DUTIES AND RESPONSIBILITIES:
Advise and assist patients with frame/lens selection.
Perform measurements of frame and lens specifications.
Customize the fit of glasses to suit client’s faces by making adjustments to the frames.
Keeping customer records up-to-date.
Ability to communicate with the public and with teammates through two way verbal and written communication.
Ability to comprehend written and verbal communication.
Ability to work with patient eyewear needs including analyzing prescriptions, repairs and adjustments, and lifestyle dispensing.
Consults with patients to resolve eye wear problems or concerns.
Maintains eye wear displays, eye wear inventory and department supplies.
Establishes contact and maintains liaison with appropriate health care professionals, institutions of higher learning, area resource agencies, and other tribal organizations.
Works individually with family members as necessary in terms of special problems and/or need related to the program
Provide monthly status reports on clients served and document source of payment.
Ensure vendors receive timely payments from other tribal program partnerships.


KNOWLEDGE, SKILLS, and ABILITIES
Knowledge of the principles of organization and management practices, diabetes-related disease processes, organizing and developing reports effectively, computer literate in work processing, Internet, and data processing methods.  Ability to meet CPR and Basic First Aid certification requirements.  Knowledge of the specific needs of Native American client at risk and/or living with complications associated with diabetes and/or cardiovascular disease.
ADDITIONAL INFORMATION/REQUIREMENTS:  Experience preferred but not required.  Willing to learn and prepare for certification within six (6) months of starting position.  Must possess a valid Oklahoma Drivers License.  Written proficiency a must.  Must be able to lift 50 lbs.  Must be willing and able to work outside of normal work hours.
