The Community Health Representative Program is funded by Indian Health Services.
Our goal as Community Health Representatives is to provide a superior quality of life to valuable members of our Tribal community.
About Our Services:
CHR provide financial relief for dialysis patients, medically necessary transportation, make home visits, monitor homebound patients, perform health screening, pick-up and delivery of medications and generally assist in meeting the medical needs of the Tribal community.
The Cheyenne-Arapaho Tribes Community Health Representative (CHR) Program has 11 CHR generalist and 1 bus route driver providing services to Tribal members within the 9 county service areas.

Program Services
Dialysis mileage Reimbursement Program:
This service provides mileage reimbursement to dialysis patients for travel to their dialysis treatments. Eligible clients are reimbursed .25 cents per mile. Prospective clients must complete the application process before eligibility can be determined. Due to minimal funding available, reimbursement can only be claimed for dialysis treatment and not for doctor’s appointments. You may contact the CHR Program to request an application by mail.
Transportation Services:
Upon request, transportation is available to area Indian Health Clinics (IHS). In addition, transportation is also provided for IHS Referrals outside the tribal service area. The CHR Transportation policy prohibits our program from transporting to outside agencies other than IHS facilities or IHS referrals.
CHR’s make their weekly schedules ahead of time, therefore, we require a 5 day notification period if at all possible. This is necessary so that CHR’s are able to accommodate your transportation request.

We keep current on each CHR’s weekly schedule and will be able to let you know if the CHR in your area is available to accommodate your request. The office staff will contact the CHR directly to relay your request directly. Every CHR is required to confirm each transportation request either by a phone call or home visit.
Home Visits:
CHR’s conduct Home Visits in their respective coverage areas to serve people in their homes; these visits are for reasons listed below, but may include others as well.
- Surgery follow up
- Vitals Check (blood sugar/blood pressure)
- Medication supplies check
- To ensure that a person is obtaining regular health care

Health Screenings:
Health Screenings are held in 9 areas every three months, while some have these screenings every other month. CHR’s conduct screenings for glucose, blood pressure, weight measurements and temperature checks, cholesterol checks can be made for high risk patients as well. A Community Health Nurse, REACH Program Staff and the Diabetes Wellness Program Nutritionist are on hand as well to assist with these screenings.
A guest speaker is invited on a specific health related topic. Nutrition and exercise information is provided to all who attend. Door prizes as well as nutrition bingo games are held to encourage a fun and friendly atmosphere. A healthy snack and drink are also provided. Everyone is invited and encouraged to attend these health out reach programs.
If you would like more information on the upcoming health screening in your area, or if you need transportation to a screening in your area, you may contact the local CHR or CHR Program. CHR’s will inform their local area of the dates and times of these projects, fliers will be posted in the local communities and in the Watonga Republican.
Medication Delivery:
CHR’s can pick-up and deliver Medications for the elderly, handicap and disabled individuals. The patient will need to call in their pharmacy medication refills prior to requesting the CHR to pick-up.
Bus Route:
The CHR Program has a bus route providing transportation four days a week to the Clinton Indian Hospital.
The Seiling/Canton route runs on Mondays and Fridays.
The Hammon/Elk City Area route runs Tuesdays and Thursdays.
To keep the mileage cost down, on the days of the bus route, the local CHR will not transport to the Clinton Indian Health Clinic. On those days clients will need to ride the bus if possible. Should you need a bus schedule please feel free to contact the CHR Program 1-800-687-0973.
